Providing transparency on how your information is store, is important to gain trust. When you provide your personal details to Charlotte Steer Chiropractic, they are stored under the Data Protection Act 2018, which includes the General Data Protection Regulation.
The reasons why I collect your personal data is stated below. All points below are considered to be a legitimate interest of yours and mine.
We must retain your records for the following 8 years after your most recent appointment or until you are aged 25, if this is longer. After this period you can request for me to delete your records. If you have not been in contact to delete your records, I will retain your records in case you decide to begin chiropractic care again.
Your personal information is always stored securely and safely. Information will be stored on a laptop which is password protected and backed up regularly. Charlotte Steer Chiropractic uses specialist medical record services which abides by the General Data Protection Regulation. Any paper versions will be stored in locked cabinets.
I will never share your personal records with anyone who does not need your information without written consent. Only myself (Charlotte Steer) and the medical records service, who stores and processes your files, has access to your records. A appointment reminder service may also be used which means that your name and email address will be saved to a server.
You will always have the right to see your personal records and make amendments if factual errors are present. You can request for your personal records to be removed if the minimum legal period of time has elapsed.
